The McKinley “Deacon” Davis CHANCE Program “50 Alumni @ 50 Awards” program celebrates the superior service, dedication and professionalism of CHANCE alumni over the program’s 50 years. As we celebrate CHANCE’s 50th year, we want to honor our alumni. This award nomination form provides the opportunity for you to nominate a CHANCE alumni who you believe has demonstrated exemplary service or professional work. We will select 50 CHANCE alumni to honor at the 50th year Anniversary Dinner Celebration Nov. 10, 2018, on the NIU campus. Please complete and submit this nomination form by Sept. 30.
